An employee was operating an ATV while inspecting vegetation in a utility right of way. The ATV was traversing a hill when the left front tire dropped off into a washout area. The ATV rolled over and landed on top of the employee. The ATV remained running as the employee worked to free himself. The hot muffler was just above the employee's back while he was under the ATV and caused third degree burns on the right side of his back.

A nurse tank was being filled with anhydrous ammonia when a valve was opened. Ammonia was released, causing chemical burns to an employee's midsection.
