An employee was in a trench that was approximately five feet deep, installing grout and bricks around a pipe that lead into a concrete drainage structure. The employee was between the concrete structure and a steel cave-in protection shield. An excavator was operating close to the trench and caused the steel shield to move, which pinned the employee's hip against the concrete structure. The employee sustained a fractured right hip.

On May 24, 2018, at approximately 10:20 p.m., an employee drove to a local site to pick up a drilling jar/tool. While the employee was waiting for the trailer to be loaded, the drilling jar (which was over 20 feet long, 6 inches wide, and weighed approximately 2,000 lbs.) rolled out of the trailer and struck the employee's right foot. The employee's right foot broke, requiring hospitalization and surgery.

An employee was attempting to open the door to a 30 yard cardboard container and lost control of the door handle after removing the locking pin. Due to the nature of the door closing mechanism (a locking fulcrum) the door handle to these types of containers can have some tension on it when opening. The container wall struck the employee resulting in blunt force trauma to his left cheek/eye socket and a broken nose.

The spool of the winch caught his glove, causing his hand to became caught in the winch. He pulled his hand back, amputating his right ring fingertip just below the nailbed.
