An employee was in a parked golf cart. He placed his feet on the ground beside the golf cart and grabbed the side of the cart to prepare his body to stand. As he stood up, he felt a pop in his right leg and fell over onto the ground. He was hospitalized with a broken femur in his right leg. His leg had a weakened bone due to a prosthetic from a previous amputation he had suffered.

A caster operator was removing a safety blank while wearing heavy welding gloves to insert a smaller nozzle in a casting tundish holding molten steel. The blank fell onto the top of the shroud, causing the molten steel to splatter onto his flame-resistant clothing and metatarsal safety boots. He suffered second- and third-degree burns to his lower legs and feet.

An employee was using a crane to lift a 1,200 lb., 36" x 18" solid copper block that was chocked with a nylon strap. The strap was not centered. When the employee lifted the block approximately 3" from the pallet it was sitting on, the block pivoted, fell from the strap, and struck the employee on his right foot. His 1st, 2nd and 3rd metatarsal bones were fractured, requiring hospitalization and surgery.

The injured employee's coworker was moving a rear suspension with a Sky-Trac forklift in the salvage yard located off of Interstate 55, Exit 71, near Blytheville. A piece of the suspension broke off and hit the injured employee in the face, fracturing his jaw, eye socket, and forehead. He was hospitalized for facial reconstruction.
