Two employees were repairing a steam leak on a pre-dryer on a 20-foot elevated surface. While welding, both employees fell approximately 20 feet when they both lost their footing on the platform. One employee fell on a peg that punctured his right thigh and was subsequently hospitalized. The second employee received a head laceration that required 9 stitches. The second employee was treated and released the same day.

When he pulled his finger out from between the two boards, the skin and tissue on his middle finger were removed. Part of his finger had to be surgically amputated.
