On September 25, 2017, at 5:00 p.m. the injured employee was loading a semi-trailer at the dock of the facility using a forklift. The truck driver pulled away from the dock with the forklift still in the trailer section of the truck. The forklift and the employee fell out of the back of the truck onto the concrete parking lot. The employee suffered a lumbar fracture and a spinal injury.
