A temporary employee was cleaning excess oil from around the air operator machine (punches a hole in material). When the employee inadvertently contacted a button located below the table, the air cylinder moved down and caught the employee's left middle fingertip between the vertical guides, resulting in fracture and partial amputation. The machine was not guarded.

The injured employee was using an oxy-acetylene torch to remove one of the legs from the outside of a centrifuge (used process equipment). After 10-15 minutes, the injured employee began having issues with the torch. He paused his work briefly and a second employee came over to assist with lighting the torch.
