On August 17, 2021, an employee was inside a trailer that his truck had just backed up to a loading dock. As he tried to straighten a banded load of cardboard, the band snapped and the load fell, knocking him into the side of the trailer. He suffered head lacerations and a dislocated shoulder.

The employee was utilizing a strap attached to the pallet when the strap broke, causing the employee to fall backwards into the forks of a fork truck. The employee suffered three fractured ribs on the right side, a bruised spleen and liver, and a laceration to the left ear, requiring hospitalization.

On October 24, 2016, at approximately 3:00 p.m., an employee was attempting to move a shutter head assemble from the shutter head machine on Line 16. The equipment was stuck. The employee went to get help and the two employees jogged the piece and the 40 pound shutter head fell approximately 18 inches. While the equipment was falling, the employee tried to catch it and his left middle fingertip was caught between the equipment and wood board resulting in amputation of the fingertip.
