An employee was operating the tire building machine at the first stage of the cycle. The employee was holding a wire bead rope material utilized in the manufacturing process. The material became caught in the machine and pulled the employee's left hand into the machine while his foot was stuck in the operation pedal. The employee suffered amputation of the left middle finger at the middle knuckle and a left ankle dislocation with a fracture.

An employee got up on a stepladder to remove debris from inside an ~8-inch by 8-inch window opening on the chill roll machine. The employee's fingers contacted the rotating parts of the machine, amputating some fingertips (no bone loss) and requiring hospitalization. The machine was not shut off at the time of the incident.

The floor was slick, and the panel slipped out from under them. The panel struck the first employee on the top of his left foot, right behind the steel toe of his boot. The big toe and second toe on his left foot were amputated.
