An employee was scraping slag off an alloy melting furnace pot when residual liquid nitrogen mixed with molten metal still in the furnace. The instantaneous mixing caused an explosion, which knocked the employee to the ground. His protective helmet and face shield were knocked off and he suffered second- and third-degree burns to the neck and left arm.

The hopper struck the employee on the left side knocking the employee to the ground. The employee sustained a right forearm contusion, two or three fractured ribs on the left side, a fracture of the left upper femoral head, a fractured vertebra, and a concussion.

On June 24, 2019, at 1:20 p.m., an employee and coworker were attempting to install a transverse compartment box weighing approximately 60 pounds in the back of a cab when the box slipped out of the employee's hand. The employee's left index finger was then pinched between the box and the wall of the cab, resulting in a fingertip amputation.
