Two employees were removing interior metal liner wall panels two at a time, leaning them against the wall, and then lowering them to the ground to be stacked. Each panel is 3' wide x 18' 6" long, and weighs approximately 30 to 35 pounds. Two panels were leaning against the wall. The employees lowered the first panel to the floor and were about to stack it, when the second panel fell and hit the injured employee’s hard hat and cut his nose and cheek. The injured employee was hospitalized.

An employee working at the magnetic particle inspection clamp in the track machine line activated a hydraulic clamp and pinched their right ring finger between the side edge of the product being clamped and the clamp wall. The pinching action trapped the employee's finger and removed the soft tissue portion of the ring finger pad under the fingernail, resulting in a partial amputation.

While adjusting the rigging for the bull gear installation, the bull gear became unstable and shifted. The shifting gear contacted the mobile platform on which the employee was standing knocking the platform over and causing the employee to fall to the ground. When the gear shifted, the employee's right index finger was caught between the rigging and the wood block resulting in amputation of the finger.
