Since the water was not coming through the wastewater treatment area, the employee determined the transfer line must be clogged, so he added steam to try to clear the line. The hose popped out of the discharge line in the pit and sprayed the employee with hot steam. He suffered burns to the wrists and abdomen.

On March 24, at around 7 a.m., an employee was working on an HVAC piping system that was under a pressure test. Believing that the pressure test was completed, he removed the end cap from the pipe. There was still pressure in the pipe, which caused the end cap to fly off and strike him in the mouth area. He suffered lacerations to his mouth and cheekbone.

The employee was removing a hose from a tank of water/oil mix that was approximately 226 degrees Fahrenheit to use in another location. The valve was open when the hose was removed resulting in the hot liquid spraying out under pressure onto the employee. The employee suffered second and third degree burns to the face, neck, stomach, and both arms.

On January 25, 2017, at around 9:30 a.m., employee 1 was operating a Skyjack Telehandler forklift with a telescopic boom and outriggers on the front. Employee 2 attached a nylon choker midway down a steel pipe that was 40 feet long and 10.75 inches in diameter. Employee 2 then attached the other end of the checker to a hook located under the boom of the forklift just behind the forks. Employee 1 lifted the steel pipe and carried it to an area where there were existing above ground steel supports
