On or about 20 April 2015 at approximately 1000 hrs an employee of Holman Well Service was hospitalized with an injured hand. The employee was assisting in the replacement of a BOP for Apache Bevins East 99-1H well head. The BOP was placed on the ground next to the well head. As the employees were working around the BOP, it fell over pinning the employee's right hand between the BOP body and the structure of the well head. The employee sustained multiple hand fractures. | |||||

An employee was stabbing up in the derrick while a pipe was being brought up on an elevator. As the employee reached to unlatch the elevator, the tongs engaged, striking the employee's left hand resulting in a broken wrist. | |||||

On or about 1555 hours on March 10, 2015, the right intermediate and distal phalanges of an employee's middle finger were amputated after his finger got caught in the pulley of a winch he was operating. The employee was holding the winch cable with his right hand due to a kink in the line and operating the winch with his left hand when his hand was pulled into the pulley. He was hospitalized and the phalanges were surgically removed. | |||||

Employee was flushing radiator on a truck and when he started to loosen the radiator hose it blew off spraying the employee with hot water. The employee suffered second degree burns on his right arm and right side. Accident happened on 2/10/15, but the employee was not admitted to the hospital until 2/11/15. |
