An employee was looking for defects in a carpet while standing on the floor between a catwalk and the exit roller of a tufting machine. He stepped backward and tripped on the edge of the catwalk, grabbing onto the exit roller of the machine with his left hand in order to catch himself. While doing so, his left index finger went into the keyway hole on the rotating exit roller, which caused his left index finger to be lacerated.

An employee was retrieving carpet rolls from a shelf located about nine feet above the ground while standing on a platform that was raised on the forks of a forklift in order to reach the shelf. Two of the platform brackets broke off causing the platform to tilt to one side. The employee lost balance and fell onto the concrete floor, resulting in a fractured left leg and a concussion. The employee was hospitalized.

The next buggy moved forward and caught the employee's leg against the conveyor fracturing his tibia and fibula near the ankle.

He was 3 feet up a 6-foot ladder, removing the stuck paper, when he fell to the cement floor, encountering wall-mounted valves, a metal catch tray on the floor, and a metal grating. He suffered back pain and two lacerations, one to the back of the head and one on the left elbow. Each laceration required stitches.
