An employee and a co-worker were conducting work on the pipeline right-of-way. The co-worker went across the bridge to help guide the employee as he drove a side-by-side utility task vehicle (UTV) across the bridge. An employee installed a nitrogen supply line containing a particulate filter, pressure gauge, and pressure relief valve for a dual gas inlet gas booster system. When the employee pressurized the supply line, the particulate filter failed, causing the quick-disconnect metal bowl guard to become airborne and strike the employee's left pinky and ring fingers, resulting in broken bones.

An employee was pulling the release rope on a self dumping hopper. As the hopper was dumping, the hopper slid forward and the safety chain separated causing the hopper to fall off the forks of the lift truck.
